How to Apply for PLBi Assistance

Nomination Procedures

The following documents must be emailed to the CSC Regional or Field Offices:
- PLBi Nomination Form (PLBi Form No. 1)
- Certification from the agency head stating:
- The nominee was killed in line of duty or died while performing official duties
- The nominee is not covered by other death benefit laws
- List of beneficiaries (with contact details and relationship), certified correct by the agency head
- Latest Personal Data Sheet (PDS) of the nominee
- Service Record of the nominee
- Medical/Police Report
- Death Certificate
Step 3. Prepare the requirements
